*BOOTH PRICE: $900 + HST (13 % Harmonized Sales Tax): The 8-ft x 10-ft booth includes wireless Internet, electrical outlet, pipe and draping assembly, one skirted table, two folding chairs, one meal planswag bag items, a punch out business card ad in the program book, plus mentions on Facebook, the OAHI website and a YouTube video. Additional power supplies and meal plans are extra. SAVE: If you book and pay for your booth before Dec. 1, 2016 the booth will cost $810 plus HST (13 % Harmonized Sales Tax). 

EXHIBITOR MEAL PLAN: Exhibitor meal plan includes welcome reception, two breakfasts, two lunches, and four coffee breaks. Additional meal packages cost $100 each plus HST (13 % Harmonized Sales Tax).

Note: 50% of the booth rental fee must accompany this application. Deposit is non-refundable. The balance and business card art are due no later than Jan 30, 2017. Booth applications will not be confirmed until full payment is received. HST (13 % Harmonized Sales Tax) is applicable for all exhibitors. HST# (868556978RT0001).

All promotional materials for the swag bag must be at the OAHI office by Feb. 09, 2017.

NEW: Please send us the names of your reps who will be staffing your booth and what days by Feb. 24, 2017. They must be on the list and must have a name badge to get meals. Your package includes one meal package per day. Additional meal packages cost: $100. Please complete the attached form.
