*BOOTH PRICE: $900 plus HST (13 % Harmonized Sales Tax): The 8-ft x 10-ft booth includes wireless Internet, electrical outlet, pipe and draping assembly, one skirted table, two folding chairs, one meal plan, swag bag items, a punch out business card ad in the program book, plus mentions on Facebook, the OAHI website and a YouTube video. Additional power supplies and meal plans are extra.
A premium booth is $1,000 plus HST (13 % Harmonized Sales Tax).
SAVE: If you book and pay for your booth before Dec. 1, 2017 the booth will cost $810 plus HST (13 % Harmonized Sales Tax). A premium booth is $900 HST (13 % Harmonized Sales Tax) if you book and pay for your booth before Dec. 1, 2017.
EXHIBITOR MEAL PLAN: Exhibitor meal plan includes welcome reception, two breakfasts, two lunches, and four coffee breaks. Additional meal packages cost $100 each plus HST (13 % Harmonized Sales Tax).

METHOD OF PAYMENT
Note: 50% of the booth rental fee must accompany this application. Deposit is non-refundable. The balance and business card art are due no later than Jan. 15, 2018. Booth applications will not be confirmed until full payment is received. HST (13 % Harmonized Sales Tax) is applicable for all exhibitors. HST# (868556978RT0001).
All promotional materials for the swag bag must be at the OAHI office by Feb. 12, 2018.

This application will not be considered binding until the Exhibitor agrees in writing to be bound by the terms and conditions of the Show Organizers (OAHI/CAHPI-Ontario) and the hotel (Four Points Sheraton Hotel London).
